We identified the root cause as a known Azure OpenAI outage/degradation affecting the GPT-4.1 model in specific EU Data Zones.
Affected Azure Regions: Sweden Central (Stockholm), West Europe, and France Central.
Currently running on GPT-4.1 UK region which is not affected.
