Intermittent Tap UI Error "Your account is not authorized to access TAP"

We experienced another wave of unexpected access issues and further mitigation measures have been implemented.
The service is now stable.
This incident is considered resolved.

The source of the issue was identified and mitigation measures were applied.
Tap UI should now grant permissions normally and users should no longer erroneously see the unauthorized access message.
We will be monitoring the situation closely.

We are investigating an issue where certain users are intermittently being shown an error message stating "Your account is not authorized to access TAP".
A possible work around may be to clear the browser's cache, then log back into Tap.
