Based on our current understanding, the issues are intermittent - retrying opening the activity at a later time is likely to help. We are seeing that some institutions/courses are more frequently affected than others. We implemented a temporary fix aimed at resolving this and are monitoring its effectiveness, while resolving the root cause.
