At approximately 11:38 pm ET on June 6th, an outage in our Azure infrastructure resulted in the inability of our users to load or submit forms. The outage was resolved at approximately 6:55 am on June 7th. All form submissions during this time were unfortunately lost, however, the forms themselves remain intact. We apologize for any inconvenience this may have caused, and thank you for your patience and understanding.
