Qualys Patch Management experienced a 53-hour incident where deployments of patch KB5123099 failed across multiple shared platforms due to a **FileHashValidationFailed** error. The root cause was an incorrect file hash, which was identified and corrected by Qualys. To complete installation, users were required to redeploy the patch via a new or re-run job, with an additional cache reset needed for systems that had cached the patch during the failure window.

This incident has been resolved.
A fix has been implemented. Now that the correct hash has been validated, in order for this patch to install, users will need to redeploy the patch by performing one of the following:
1). Create new job with the patch included
2). Re-run of the previous job created
3). cloning the old job and execute
If the Patch is cached, reset will be required as the cached patch will have the same download URL when the error was first observed and continue to fail install.
Once the patch is redeployed, hash validation and patch install will be successful
The issue has been identified and a fix is being implemented.
