1Password experienced a sign-in issue where the 2FA prompt failed to appear for users who changed their password and had 2FA enabled, or when the Restricted Failed Login Attempts policy was enabled with 2FA enforced. As a temporary mitigation, the Sign-In Attempts policy was disabled for all affected users during the incident. The issue was permanently resolved with the release of version 8.12.4 after approximately 39 days.
