DAX Copilot for Dragon Medical One (DMO) experienced a major incident lasting approximately 3 hours on August 13, 2026, during which clinicians were unable to receive completed AI-generated summaries — summaries either remained in a "writing" status, disappeared after upload, or were significantly delayed. Recordings themselves were not affected and no data was lost. Microsoft identified and mitigated the root cause, then successfully reprocessed all impacted encounters, restoring full summary availability for both affected and new recordings.
